For a Health Information Exchange (HIE) to be successful, patient summaries should be shared electronically. When a doctor gets a medical summary from an outside source (ie: hospital, another physician, or a patient) the differences must be reconciled with what is in the EMR/EHR-- usually some time-consuming,manual and error-prone process to discover the differences.
Present a secure embeddable web-based user interface to a clinician to allow interaction with an intelligent comparison tool that lines up similar records to easily see and reconcile differences. Support multiple electronic formats (CCR, CCD, CDA, HL7) and a variety of identifiers (SNOMED-CT, RxNorm, FirstDataBank, CPT, ICD9, MetaThesaurus). Finally, transmit the reconciled results electronically back to the EMR/PHR.
How it works
FroozHIE is a secure website that can be launched from any EHR or PHR application. The EHR or PHR application passes an incoming summary document and a destination summary document as CGI parameters. A URL of where to post the results is also passed along with the requested format for the results. A web browser shows an AJAX based web interface to allow the user to interact.

Continuity of Care Record (CCR)
HL7 Continuity of Care Document (CCD)
CCD files should conform to the C32 constraint of the standard. NIST has a test tool to validate CCD files. It is recommended that the files are valid up to "HITSP/C32 v2.5 Summary Documents Using HL7 CCD".
