Meridian is an innovator in patient-centered healthcare IT, and is integrating the PHR into its medical home initiatives. For example, Meridian is pilot testing MD Mouse, a computer mouse with an on-board blood pressure monitor that allows patients to monitor and upload BP readings directly into the PHR.

The Meridian PHR enables patients to create a personal health record account and share it with Meridian physicians. Patients can access their personal health records on the web anytime, anywhere. Patients who travel or live part-time outside the Jersey shore can also send their PHR to physicians around the country.

Meridian patients will also be able to print out an NMC911 card that contains emergency information including medications, allergies and current conditions. Patients can also select a four digit PIN number that will give emergency responders read-only access to the PHR.
