Meridian Health Case Study |
 Meridian Health in New Jersey recently launched an online patient portal that features a Meridian branded version of the NoMoreClipboard PHR. Meridian operates four hospitals at the Jersey shore. Meridian is an innovator in patient-centered healthcare IT, and is integrating the PHR into its medical home initiatives. For example, Meridian is pilot testing MD Mouse, a computer mouse with an on-board blood pressure monitor that allows patients to monitor and upload BP readings directly into the PHR. The Meridian PHR enables patients to create a personal health record account and share it with Meridian physicians. Patients can access their personal health records on the web anytime, anywhere. Patients who travel or live part-time outside the Jersey shore can also send their PHR to physicians around the country. Meridian patients will also be able to print out an NMC911 card that contains emergency information including medications, allergies and current conditions. Patients can also select a four digit PIN number that will give emergency responders read-only access to the PHR. "NoMoreClipboard has been great to work with," according to Sandra Elliott, Meridian Health Director of Consumer Technology and Service Development. "They are able to integrate with other applications, devices and programs -- making the Meridian-branded PHR the communication hub between our patients and our healthcare delivery team." |

Bon Secours Health System |

 The Bon Secours Health System in Virginia is employing a progressive approach to communicating with its patient population. This health system created mybonsecours.com, an online patient portal that enables patients in Richmond and Hampton Roads to find a physician, search for online health content, check symptoms and perform financial transactions.
At the heart of the portal is a mybonsecours personal health record created by NoMoreClipboard. This PHR is branded to Bon Secours, and retains the same look and feel as the rest of the mybonsecours portal. The PHR is sponsored by Bon Secours, and enables patients to create an online PHR where they can compile, manage and share their health information. From the PHR, patients can quickly find a Bon Secours physician and send their PHR to that physician in advance of a medical visit. The mybonsecours PHR completes and delivers the registration forms used by Bon Secours practices, so that patients can avoid filling out a clipboard full of medical questions when they arrive for an appointment. The mybonsecours PHR is also integrated with online health content. Patients can search for more information on medications, illnesses and conditions entered in their PHR, and are linked to health content from the Healthwise Knowledgebase. The mybonsecours PHR will also be integrated with Bon Secours practice management systems, electronic medical record systems and the MedVirginia health information exchange. |
