Why Now?Changing Times: While most U.S. physicians still use paper charts to manage patient records, doctors and hospitals can now qualify for government funding to implement an electronic health record (EHR). As doctors put in EHR systems, they can provide better care, reducing medical errors and duplicated services. To qualify for these funds, healthcare providers must demonstrate “meaningful use” of a certified electronic health record system. One of the meaningful use requirements is that doctors and hospitals must provide patients who ask with an electronic copy of their medical information. For an EHR system to be certified, it must be able to generate a standard electronic record called a continuity of care document (a CCD). Over the next few years, most doctors and hospitals are expected to convert from paper to electronic records. As they do, they will can provide patients with electronic copies of their health records in a CCD format. That’s where cc:Me comes in – giving those who provide care a quick and easy way to share health information with you. How cc:Me worksPersonal cc:Me address: Once you have a NoMoreClipboard personal health record account, you can create your own cc:Me web or email address. You will be prompted to enter a personalized “handle” to complete the address – www.ccme.com/”your handle” or “your handle”@ccme.com. Sharing: There are several ways to share your cc:Me address. You can print out a “Hey Doc, cc:Me” instruction sheet that you can share with a physician or hospital. This guide will include your cc:Me address and instructions on uploading an electronic document to your personal health address. You can also print out an NMC911.com wallet card that can give emergency responders read-only access to your personal health record. You can have your cc:Me address printed on the NMC911.com card for easy retrieval. Your doctor’s office or hospital can simply enter your cc:Me address where they can upload or post the CCD version of your health information. It’s that simple. What will my doctor say?Paper or Computer: First of all, your doctor may still use paper charts. Not a problem, as the “Hey Doc, cc:Me” instruction sheet can also be used as a fax-back cover page. Your doctor can use it to fax paper medical records, lab reports and other documents directly to your NoMoreClipboard in-box. When you print out a “Hey Doc, cc:Me” guide, it includes a unique bar code tied to your NoMoreClipboard account, so whatever gets faxed is delivered right to your personal health record. If your doctor has an electronic health record system or is in the process of implementing one, cc:Me should be a welcome addition. Your cc:Me address gives a medical practice or hospital an easy way to share electronic records with you – helping them qualify for incentive funds. While they may have questions, the “Hey Doc, cc:Me” instruction sheet will explain how everything works. Communicate with your Physician: We don’t expect cc:Me to be an overnight success – not every doctor will be ready or able to send electronic medical information to your NoMoreClipboard account. However, nearly every office has a fax machine, and faxing records to your personal health record is faster and easier than making copies of your health information. When you start a dialog with your physicians, hospitals and other care providers, they will likely take action. The number one reason doctors and hospitals contact us to learn more about NoMoreClipboard products is because their patients asked them to. With your help and the help of other NoMoreClipboard users, we can simplify the way health information is shared. Over the next few years, this effort can help improve health outcomes and reduce medical costs. |
